Main Function

The post holder is responsible for managing their own community caseload, supporting people with substance misuse issues, in their own homes and other community venues. The post holder will provide support and education to significant others and families of people who misuse substances. The post holder will promote positive and equitable engagement with clients, significant others and families.

Main Duties

  1. Responsible for the care management of own caseload by assessing need, delivering specialist intervention, evaluating the effectiveness of these interventions and ensuring ongoing care planning and support.
  1. Work in collaboration with statutory and voluntary agencies to ensure a co-ordinated approach is developed to benefit clients.
  1. Ensure highest standards of service delivery and demonstrate best practice.
  1. Act in an appropriate manner to ensure that the work of CrossReach is seen positively and that its Christian ethos is maintained.
  1. Be aware of implications of all clients’ risk assessments.
  1. Assist clients to have a high quality of life and facilitate them to have choices.
  1. Responsible for own professional development and participation in continuing education, to ensure evidence based practice.
  1. Maintain high quality, contemporaneous written records, and provide written reports as required.
  1. Contribute to case conferences, reviews and other meetings relevant to your work.
  1. Ensure that the requirements of your professional registration bodies are met and registration is current if applicable.
  1. Given the internal requirements for flexibility, undertake skill development in other compatible areas of work within the service when required.
  1. Be familiar with the Service’s policies and procedures and ensure they are applied effectively.
  1. To undertake other duties as may reasonably fall within the parameters of the post as delegated by Managing Co-ordinator/Service Leader.
  1. Have respect for all aspects of Christian worship.
  1. Facilitate Service Users to attend Christian worship (or worship of choice as required by the role).

Qualifications

Desirable: A recognised Social Care Qualification (SVQ L3)

SSSC registration – to be achieved within relevant timescale as applicable.

Personal

Have a non-judgmental attitude to people with substance use issues.

This post requires you to be a member of the Protecting Vulnerable Groups (PVG) Scheme and undertake the necessary vetting checks.

Skills and Abilities

Essential:

  • Postholder must possess basic computer literacy skills including use of Microsoft Office and the internet.
  • Full driving licence.

Desirable:

  • Previous experience in substance use field.
  • Knowledge of CRAFT.
