Main Function

To work as a team member providing support appropriate to the needs of the service user within this Housing Support service.

Main Duties

  1. To work as part of a team involved in the general day to day support of the service users as determined by the Unit Manager and Team Leaders.
  2. To create and maintain a safe and secure environment in the unit that is welcoming and friendly for all who use it.
  3. Maintain the standard of health and safety with in the unit:
    • Report accidents and unusual incidents immediately to senior staff.
    • Carry out regular checks on the building to make sure it is safe with regard to fire, flood, etc.
    • Carry out checks on the wellbeing of the residents, with due regard to their privacy.
    • Monitor residents' physical and mental health, reporting changes to senior staff immediately and responding to such changes appropriately.
    • Facilitate residents to access health care appropriate to their needs.
    • Encourage residents to maintain a high standard of personal hygiene.
  4. To develop your knowledge and understanding of the needs of service users and how you can best support and assist them. To undertake any training essential to your professional development in fulfilling the role of a Support Worker as determined by senior staff.
  5. To gain a knowledge and understanding of, and to liaise with, a wide range of outside agencies that may be of help in meeting a service users’ needs
  6. To Support and assist service users to have a high quality of life by:
    • Promoting choice in service users ' lives.
    • Encouraging and facilitating the development of personal interests.
    • Encouraging service users to participate in the community and fulfil normal activities.
    • Helping service users with reading and writing when required and with filling out forms
  7. Involvement in the daily running of the duty office - receiving phone calls and referrals, contact with service users.
  8. Make and keep appropriate records of all meetings with service users and to record significant events involving each service user.
  9. Interview clients for admission under the guidance of senior staff.
  10. To be key worker to a number of service users as determined by senior staff. Organising key work and review meetings with service users.
  11. Contribute to the setting up of Personal Support Plans with key clients. This involves:
  12. Working closely with service users to make an assessment of their needs
  13. Encouraging active involvement by service users in drawing up and carrying out plans
  14. Writing up of Personal Support Plans
  15. Involvement in their implementation
  16. Careful observation and regular reviewing of service users ' needs and
  17. Responding to service users ' changing needs and circumstances
  18. Storage of medication for some service users to self administer in line with unit policy and procedures.
  19. This is done under the supervision of senior staff.
  20. Contribute to case conferences, reviews and other meetings. Compiling of appropriate reports and records for such meetings.
  21. Participate in regular supervision with senior staff.
  22. Participate in shift changeovers, team meetings and weekly staff meetings.
  23. Work on a rota basis involving day, evening, weekend and occasional overnight cover.
  24. Assist with cooking and cleaning in the unit if required.
  25. The post holder will be expected to undertake other relevant and reasonable duties as required by senior management.
  26. Have respect for all aspects of Christian worship.
  27. Facilitate service users to attend Christian worship (or worship of choice).

Qualifications

Essential:

A recognised Social Care Qualification (minimum SVQ L2 or equivalent) and must be willing to complete SVQ L3 within 1 year if not yet qualified.

Desirable:

Addictions Qualification, Certificate in Counselling Skills.

Skills and Abilities

Essential:
