Your Individual Care Plan
When you come to Well Hall, you will be allocated a keyworker who will discuss and develop a bespoke care plan with you. This records your ongoing needs, preferences and aspirations, so all staff know how best to support you.
Well Hall offers open visiting, and staff are happy to have relatives share in the care of their relatives. We also welcome and appreciate your participation in organised activities such as outings, events and religious services.
We actively encourage residents to continue with activities and interests they had before coming to Well Hall. We will do our best to help you maintain links to the community and any other external agencies as appropriate. We also invite others into Well Hall, whether volunteers, local community groups or our chaplain. Our Friends Group visits the service and organises fundraising events for the benefit of all who live and work at Well Hall.
Whatever you want to do, we will try to make it happen. We can provide materials necessary for specific activities in the house, and we can arrange shopping trips, outings and visits to places of interest.
